Prepare fish
Pat the grouper fillets dry with paper towels. Cut into 4 roughly equal sandwich-sized portions (about 4 oz each). Season both sides evenly with 1 tsp salt, 1/2 tsp black pepper, 1/2 tsp garlic powder, and 1/2 tsp paprika.
Set up dredging station
Place 3/4 cup flour in a shallow dish. In a second shallow dish, beat 2 eggs with 3 tablespoons milk until combined.
Dredge fish
Working one piece at a time, lightly coat each fillet in the seasoned flour, shake off excess, dip into the egg wash, then press back into the flour for a light double-coating. Set on a plate to rest for 5 minutes so coating adheres.
Pan-fry fish
Heat a large skillet over medium-high heat and add 1/2 cup vegetable oil. When oil is shimmering but not smoking (about 350°F / 175°C), add fillets without crowding the pan. Fry 3–4 minutes per side, until golden brown and internal temperature reaches 140–145°F (fish flakes easily). Adjust heat as needed to avoid burning; you may need to fry in two batches. Transfer cooked fillets to a wire rack or paper towel-lined plate and let rest 2–3 minutes.
For the sauce
While the fish rests, make the sauce. In a small bowl combine 1/2 cup mayonnaise, 1 tablespoon Dijon mustard, 1 tablespoon fresh lemon juice, 1 teaspoon chopped capers (if using), 1/2 teaspoon hot sauce (if using), 1/4 teaspoon salt and 1/4 teaspoon black pepper. Whisk until smooth and taste; adjust lemon, salt or hot sauce to preference.

Assemble sandwiches
Spread about 1 tablespoon of the sauce on the bottom half of each bun. Place a lettuce leaf on the sauce, then a fried grouper fillet. If using cheese, place a cheddar slice on the hot fillet so it softens slightly. Top with a couple of tomato slices and a few thin rings of red onion. Spoon another 1 tablespoon of sauce on the top bun and close the sandwich.
