Prepare juices
If not already prepared, squeeze fresh orange juice and fresh pineapple juice. Strain out any pulp for a smooth texture.
Combine ingredients
Add 2 tablespoons (approximately 1 oz) dark rum, 3 oz fresh orange juice, 3 oz fresh pineapple juice, and 1.5 oz cream of coconut into a cocktail shaker.
Add ice and shake
Fill the shaker with about 1 cup cubed ice. Secure the lid and shake vigorously for 12–15 seconds until the outside of the shaker is frosty and the mixture is well chilled and slightly aerated.
Strain and serve
Strain the cocktail into a chilled Collins or highball glass filled with fresh ice (or serve neat in a coupe if preferred).
Finish with nutmeg
Grate a light sprinkling (about 1/8–1/4 tsp) of fresh nutmeg over the top of the cocktail to release aromatic oils. Use the remainder of the 1 tsp for additional garnish or other servings as desired.
Garnish
Garnish the glass with an orange wheel or wedge on the rim and optionally tuck a pineapple leaf or small pineapple wedge beside it. Serve immediately.
